Resilience

Primary health care providers have had to be dynamic and adaptable in the face of significant challenges over the last few years. 

Through trying times, SEMPHN and commissioned partners including General Practitioners and other service providers have continued to serve the community. It has been a tough few years with the implications of the pandemic on health care and the drain on its workforce, both nationally and internationally. 

We can be proud of our collective resilience and how we have worked as communities and stakeholders to make the necessary changes moving into the pandemic. The same spirit of commitment and support will guide us as we re-emerge and redefine our work in the coming years.  

We’ve collaborated and worked together with General Practice, hospitals (our Local Health Networks), public health services, Allied Health providers, Aboriginal Community Controlled Health Organisations, Residential Aged Care Facilities, Rehabilitation Facilities, and so many more community-based health service providers to guide our consumers and communities to access the best primary health care in south east Melbourne.

Support

SEMPHN exists to ensure Victorians in south east Melbourne have access to health services which are effective and integrated across primary, acute and aged care. 

We have continued to provide not only funding to stakeholders through commissioning to allow them to change tactics and recover quickly from the impact of COVID-19, but also in our role as connector and facilitator; through our strong relationships and a deep understanding of the south eastern Melbourne community. 

The past year has also seen SEMPHN deliver support and information services to local government and active engagement across our teams and programs that support our communities.

Evolution

We are adaptive and are evolving to the unique needs of primary healthcare within one of the most diverse communities in Victoria.

Adapting and responding to a complex and rapidly changing health landscape is integral to the success of primary health care providers. With this in mind, providers within our catchment focus on promotion and prevention and addressing health determinants using a people-centred approach. We work with residential aged care facilities, pharmacy providers, general practice and local health networks to evolve our collaboration and to deliver person-centred primary health care.

Community

South Eastern Melbourne Primary Health Network (SEMPHN) exists to assist primary health care providers to deliver the best care in the community.

SEMPHN prides itself on having the insight and deep understanding of our community’s health needs and the ability to connect consumers with the right care – often in a fragmented health system. Through our extensive Needs Assessments and robust commissioning processes, we identify needs and service gaps and then integrate care solutions to meet the needs of the community, providers and the health system. 

Our teams work with a diverse range of service providers in local communities and co-design evidence-based solutions which deliver value and positive health outcomes.
